Significant Financial Interests (SFIs) Disclosure
The PHS FCOI regulation requires Investigators (as defined by PHS guidelines and includes spouse and dependent children) to disclose the occurrence of all Significant Financial Interests (SFIs) at the time of PHS application
1. Complete the Significant Financial Interests Form (SFIF). Reference ALL applicable PHS awards & applications. ORA will match the disclosure for you.
2. For each SFI, complete the SFI Attachment
3. Sign and submit to firstname.lastname@example.org

4. If you acquire a new SFI during a PHS award period, please submit a new attachment. You do not have to resubmit the SFIF.
The PHS FCOI regulation requires Investigators to disclose the occurrence of any reimbursed or sponsored travel (i.e., that which is paid on behalf of the Investigator and not reimbursed to the Investigator so that the exact monetary value may not be readily available), related to the Investigator’s institutional responsibilities.
Disclosure for each travel event must be made within 30 days of reimbursement or occurrence of sponsored travel
