PHS Travel Disclosure Form

This form should be completed after travel that is paid for or you are reimbursed for by any entity not listed in the exlusion list.  Please list all projects that are applicable.

ORA requires that this form be completed by all consultants deemed to be investigators, as well as subrecipient investigators, prior to proposal submission.

If the individual or organization elects to follow OUHSC’s COI Policy on specific PHS-sponsored projects, and specific projects sponsored by organizations that have adopted PHS regulations (e.g., American Heart Association, Susan G. Komen Foundation; a full list can be found here). ORA will require that they complete training and this form prior to submission.
